Medication consent

Any parent/carer requesting the administration of medication should read the policy 'Supporting Pupils with Medical Conditions' and complete this form.

The school will not give children any medication unless this form is completed and the SENCo or a school leader has confirmed that school staff have agreed to administer the medication.
